If you prefer to register online, go to www.eayso.org and follow the instructions. Then print ,sign, and mail forms with check to:

Be sure to identify your child on the check.
Or bring the forms to the mall during Open Registration.

Costs
Costs are as follows:
$45 for the first child in the family
$35 for the second
$25 for any more in the family.
If a uniform is needed, please send a separate check, including legible player name and sizes needed. Uniform costs are $18 for the jersey set (home and away), or $25 for jersey set, shorts, and socks.
Uniforms will be made available during open registration and the first week of practice.
Munchkins
For the Fall season players must be at least four years of age by August 21, 2011. Munchkins meet one evening per week for a combined practice and game. You will be contacted by your child's
coach with details.
The cost is $45 and includes a T-shirt.
To Volunteer:
Fill out a Volunteer Application www.eayso.org and mail or bring it with your child's registration. Then call 980-7789 to let us know how you prefer to volunteer your time.
